1. Copy of Lease agreement or closing papers if owner of home.
2. Copy of Photo I.D.
3. Pedigree Information on everyone that will be living in the home.
4. Name, date of birth, last 4 numbers of SSN and birth certificate of all minor children under 17 years of age.
5. No permit will be issued without current proof of trash service in the owner’s name.

What is needed for a City Permit?

1. The fee is $100.00 (money order only)
2. Recorder of Deed or Proof of ownership dwelling.
3. If a property management company rep., a letter of agreement is required.
4. Photo I.D. is required.
5. Proof of Trash Service in owner’s name only, per City Ordinance 819.
6. Current paid Real Estate Tax receipt.
7. Current HVAC report from a Certified Inspector licensed by St. Louis County.

You can obtain an accident report through CRASHDOCS.org simply go to CRASHDOCS.org
